ONIN Referral and Delegation Resources

Below you will find all of the documentation required to send a referral within ONIN. For each type of referral, you will need to ensure you include the following with as attachements to the Ocean Referral Form:

General Consent Forms:
1) Patient Consent for Type of Referral (Prescription, Medication or Delegation)
2) Email Consent Form
3) Payment Authorization Form
Relevant Clinical Context for the referral, including any assessments that are relevant
Any other important information about the patient that you wish the NP to be aware

Lab Assessment Referrals

If referring a patient for consideration of specialized lab testing, please include the three forms below along with your clinical reasoning and rationale for the specific test(s) requested. Please note that these tests are generally not covered under OHIP and are the patient's financial responsibility — ensure this has been discussed with the patient prior to referral.
